Program: Jumpstart Program (Burnout Buster)

Coaching begins:   Upon receipt of first payment. After registering, please schedule your first session at https://healthcarecoaching.coachesconsole.com/calendar/40-minuter-established-client     At the first session, you can decide if you want to schedule your following sessions or if you want me to schedule them for you at that time. 

Coaching completes: After 8 sessions or four months, whichever comes first

As your coach, I want to congratulate you for investing in yourself in a way you may never have before. You’ll see from what’s included in this coaching experience that it is designed to deliver an extremely high level of accountability and support, so that you will make empowered decisions and take bold action toward achieving your goals.

 Saying “Yes” to this experience means showing up for yourself in a whole new way, mentally, energetically, spiritually and physically. This is the key to achieving everything you want and everything you were meant to be; so is asking for support and being open to new ways of thinking. Plus, being willing to be challenged to go outside your comfort zone will help you make your personal growth that much faster.

 In order for you to get the most out of your coaching program, you must agree to make the following commitments:

 YOUR COMMITMENT TO YOURSELF AND THE PROGRAM INCLUDES:

• Reading, signing and returning this Agreement to me prior to your first session.
• Making the agreed-upon payments in a timely manner
• Attending all coaching calls on time and refraining from multi-tasking during calls
• Being authentic, honest, and speaking up for what you want
• Letting me know about any personal situations that may interfere with your experience in the program.
• Letting me know if there is anything you don’t like about how the coaching is going
• Be willing to take risks, try new things and stretch beyond your current experience
• Being responsible for your own results, which includes proactively asking for support, scheduling coaching calls and using your program benefits.
• Honoring the investment you are making in yourself by doing the work, including any assignments that you take on.
• Celebrating your wins, shifts, achievements and excitement throughout the program

MY COMMITMENT TO YOU INCLUDES:

• Seeing you as naturally creative, resourceful and whole, and infinitely capable
• Believing in you and your ability to achieve your vision of success
• Using coaching skills and techniques from a variety of disciplines with you so you can move forward with your goals much more quickly than you would on your own

 SCHEDULING:

• The number and length of your coaching sessions is determined by the program you select.
• Except for the jumpstart session and wrap-up session, you will be responsible for scheduling your other coaching sessions using my scheduling software. You should schedule your sessions so there is approximately 14 days between sessions, which will allow you to integrate the learnings from each session.
• Please arrive at your sessions prepared and on time. The ending time of the session cannot be extended because there may be other client sessions following yours.
• Your program expires on the date noted in the contract even if you have not used all your sessions.

CANCELLATION POLICY:

• You receive one “emergency reschedule” in your program, which means you can cancel a session within 24 hours and still reschedule the session. This reschedule must be done by email or telephone since my software does not allow rescheduling within 24 hours of the appointment
• Any other canceled sessions with less than 24-hour notice will be forfeited

 INVESTMENTS AND REFUNDS:

The investment in the program is $2395 in full, or four monthly installments of $629

In the event that you decide your purchase was not the right decision, within 30 days of enrollment, contact me at gstrichmd@gmail.com and let me know you’d like a refund, by the 30th day at 11:59 EST.

Notes about our refund policy:

Within the first 30 days from original date of purchase, you can request a refund of your original investment.

No refunds will be given after 30 days from the original date of purchase. After day 30, all payments are non-refundable, and you are responsible for full payment of the fees for the program regardless if you use the remaining sessions or not. If payments are not made on time, you agree to pay interest on all past-due sums at a rate of 1.5% per month. All refunds are discretionary as determined by the company.

If Client elects to pay by monthly installments, Client authorizes the Company to charge the authorized charge card on the first day of each month in the amount described above or until the total agreed-upon investment is made.

The Coach may terminate this agreement at any time if he/she decides that continuing the coaching relationship would not be beneficial either to the Client or to the Coach. In that case, a refund will be made prorated to the number of sessions not used.

DISCLAIMER

As with anything in life, your results may vary and will depend on several factors including your experience, expertise, motivation, commitment, and actions. I do not make any guarantees about your ability to gain results or gain future earnings with the strategies, information and/or ideas taught in coaching sessions or any training. You are responsible and accountable for your own decisions, actions and results.

Coaching does not involve the diagnosis or treatment of mental disorders as defined by the American Psychiatric Association. Coaching is not a substitute for counseling, psychotherapy, psychoanalysis, mental health care or substance abuse treatment and should not be used in place of any form of diagnosis, treatment or therapy.  If you are currently in therapy or otherwise under the care of a mental health professional, as your Coach, I encourage you to consult with your mental care provider regarding the advisability of working with a coach.  Do not discontinue or change any treatment plan that you may be on as a result of our sessions without discussing the change with your doctor/provider.

Coaching is not to be used as a substitute for professional advice by legal, medical, financial, business, spiritual or other qualified professionals. The client will seek independent professional guidance for legal, medical, financial, business, spiritual or other matters, and understands that all decisions in these areas are exclusively theirs and acknowledges that any decisions and actions regarding them are their sole responsibility.

 CONFIDENTIALITY

As your coach I will keep all of our coaching conversations confidential to the extent allowed by law. I record or transcribe coaching notes of all sessions and these will be kept confidential unless you explicitly agree to allow me to share anonymized records with an ICF approved mentor coach for purposes of mentoring or credentialing. Your name and contact information may be shared with the ICF for purposes of credentialing only.

Under California law, I am obligated to break confidentiality only under the following circumstances, or under court ordered subpoena:

• If you divulge in a coaching session evidence of child or elder abuse.

• If in my presence you threaten to kill or harm another individual, and I am convinced that you will act on this threat, or that you may lose control of your actions.
• If at any time during the course of our sessions, I determine that you are a danger to yourself, I will inform you of that opinion and make every effort to keep you from endangering your life.  In some cases this may include notifying the police, your healthcare professional or family members.

From time to time you may choose to accept assignments or exercises designed to help you make progress between sessions. Many of these exercises are copyrighted and I am licensed to use them with my clients, but you may not copy or redistribute them to others.

USE OF AI IN COACHING SESSIONS

Please be aware that I use AI-generated transcripts and summaries of all my coaching sessions for my own personal use. The app that I use for recording has HIPAA-level compliance. I use AI to review my session transcripts and help me prepare for upcoming coaching sessions. I have set the security parameters on all of my AI programs to not use any identifiable client information in training their large language models.

 IMDEMNIFICATION AND OTHER LEGAL STUFF

You acknowledge and take full responsibility for your life and well-being, as well as the lives and well-being of your family (where applicable), and all decisions made during and after the program.  By signing this contract you agree to indemnify, hold harmless,  waive, release and discharge Healthcare Coaching Associates LLC, myself, my spouse, my heirs, executors, administrators, assigns, officers, agents, employees, representatives, and all others acting on their behalf (the “Released Parties”) from any and all claims, damages obligations, losses, liabilities, cost or debt, and expenses (including but not limited to attorney’s fees), to your person and/or property or that of your family (where applicable), including those caused by negligent act or omission of any of those mentioned or others acting on their behalf, arising out of or connected with your participation in the Program or in connection with services provided by myself or the Released Parties.

This Agreement may not be modified without the prior written consent of Client and Coach. The waiver by either party of a breach, right or obligation shall not constitute a waiver of any other or subsequent breach, right or obligation.  If any provision of this Agreement is found to be invalid or unenforceable for any reason, the remainder of this Agreement shall remain in full force and effect. This Agreement shall be construed and interpreted in accordance with the laws of the state of California.
